Healthy at Every Chapter of Life

Carroll County faces some of the highest rates of chronic disease in New Hampshire — and as the oldest county in the state, with a median age of 54, the stakes are high. C3PH is committed to shifting our community’s approach to health from reactive to proactive: catching problems early, connecting residents to preventive care, and supporting healthy aging across all stages of life.

Prevention and Screening: Addressing Chronic Health Conditions

Prevention and screening are essential for detecting and managing chronic health conditions before they progress. They encompass physical check-ups, early intervention, and informed health decisions, ensuring a longer and healthier life.

Helping Carroll County start strong and stay strong through prevention and screening.

Cancer Screening and Adult Preventive Care

Cancer is the leading cause of premature death in Carroll County. The good news is that many cancers — including breast, colorectal, and cervical cancer — are highly treatable when caught early. C3PH works with local healthcare partners to promote and expand access to cancer screenings and routine preventive care, bringing resources directly into our dispersed communities rather than expecting residents to navigate the system on their own.

Reducing Cancer and Cardiovascular Disease

Cancer and heart disease together account for the two leading causes of premature death in our county. Addressing them requires the same proactive approach: routine blood pressure, cholesterol, and diabetes checks can identify risk before it becomes crisis. C3PH works to link chronic disease management resources to primary care, helping residents manage their health before conditions escalate into serious — and costly — health events.

Healthy Aging, Social Connection, and In-Home Support

With 33% of Carroll County residents over the age of 65, supporting healthy aging is one of our most pressing public health responsibilities. C3PH supports initiatives that help older adults stay connected, independent, and healthy at home.

Pediatric Well Visits and Developmental Screening

Healthy children grow into healthier adults. Routine well visits are about much more than vaccinations — they are critical opportunities to screen for developmental delays, behavioral health concerns, and other conditions that are far easier to address when identified early. In a rural county like Carroll County, access to pediatric specialists can require long travel times. By supporting consistent well-child care and developmental screening close to home, C3PH aims to ensure that every child gets a strong start, and that families get the support they need along the way.
