Comprehensive Support for Substance Use Disorder and Mental Health

Carroll County Responds to Substance Use Disorder – A Community Collaboration to Promote Prevention, Intervention, Treatment and Recovery in our community.

Background

The NH Department of Health and Human Services/Bureau of Drug and Alcohol Services (DHHS/BDAS) has determined that the best way to prevent and/or decrease the damage that substance misuse causes to individuals, families, and communities is to develop a robust, effective and well-coordinated continuum of care in each region of the state, and to address barriers to awareness and access to services. Regional continuum of care development work is carried out within Regional Public Health Networks by a full time Continuum of Care Facilitator. This work is guided by and documented through the deliverables as outlined by the Regional Public Health Network contract with the BDAS. For more information go to dhhs.nh.gov.

Overview

Carroll County Coalition for Public Health(C3PH) provides leadership and coordination to develop a comprehensive, effective, and well-coordinated Continuum of Care (CoC) for substance use disorders. This work includes convening subject matter experts and stakeholders to identify assets and gaps and to develop regional plans that initiate cooperative actions that increase awareness of current services, remove barriers to accessing services, increase capacity of existing services, and develop new services to fill gaps.

C3PH has completed an asset and gaps scan to identify resources, gaps and barriers that impact regional capacity and can help or hinder the achievement of the region’s statement for continuum of care development. The process for identifying regional assets and gaps included one-on-one meetings, community meetings and events, focus groups, key informant interviews and staff research. There were a significant number of gaps and barriers identified, as well as some various needs related to communication and collaboration across the continuum. Assets were identified using the Carroll County Substance Misuse Treatment Program Resource Guide, Carroll County Resources Directory website, the nhtreatment.org website, and through significant community knowledge and input. Continuum of Care Facilitator: Roles and Responsibilities

The role of the Continuum of Care Facilitator is to bridge the components of the Substance Use Disorder (SUD) Continuum of Care in their respective networks, including health promotion, prevention, early identification and intervention, treatment, recovery supports and coordination with primary health and behavioral health care. The role of the COC facilitators is achieved through the following activities:

Ongoing community organization and relationship building among stakeholders representing the full continuum of care within the region
Providing information and resources (including information such as stigma reduction strategies and skills such as evaluation and grant-writing) in service to improve systems of care and integration of behavioral and medical healthcare
Convening, facilitating and attending opportunities for collaboration (meetings, events) with regional SUD experts and service providers

Community Health Improvement Plan (CHIP) Alignment

Aligning with the Community Health Improvement Plan

The Carroll County region has identified Access to Comprehensive Behavioral Health Services and Substance Misuse & Addiction as two of the six health priorities in its Community Health Improvement Plan (CHIP), entitled Carroll County Promise 2020. The Access to Comprehensive Behavioral Health Services CHIP priority area is a key component of the Continuum of Care initiative.

Goal: Improve access to a comprehensive, coordinated continuum of behavioral healthcare services in Carroll County by 2020.

Objectives

  1. Increase awareness and decrease stigma of behavioral health issues.
  2. Increase collaboration and education with agencies, businesses, individuals and the general public to address specific issues, including: suicide, depression and feelings of hopelessness and isolation.
  3. Reduce rate of suicide deaths and suicide attempts by adolescents each year (NH SHIP).
  4. Reduce number of hospital emergency department visits for mental health.
  5. Improve access to mental health screening, prevention, and early intervention for residents through primary care settings.
  6. Increase access to affordable health insurance coverage.

Priorities for 2020-2021

To build the capacity of and expand the delivery of services in the region
To improve communication and build collaboration among service providers
