Jennifer Thomas
Substance Misuse Prevention Coordinator
Jen Thomas is the Substance Misuse Prevention Coordinator at Carroll County Coalition for Public Health. Jen has a Master of Education in Health Promotion and Health Education from Plymouth State University and is a Certified Prevention Specialist (CPS), an international certification reflecting her training in evidence-based best practices for preventing substance misuse. Jen has also trained extensively on suicide prevention, including how to improve risk and protective factors. Jen also facilitates the Carroll County Veterans Coalition, which aims to reduce veteran suicide and provide increased supports and resources for service members, veterans and their families. Jen has lived in and enjoyed the community she serves with her husband and son for many years but is originally from Massachusetts. Her husband is almost 4 years into his recovery and is 100% substance free. This family experience sparked her desire to go into prevention to provide support to other families. Linda Burns
Citizen Corps Volunteer Coordinator
Linda Burns is our Volunteer Coordinator for Carroll County Citizens Corps. This group includes the Community Emergency Response Team (CERT) and the Medical Reserve Corps (MRC). The mission of these volunteer teams includes community education, training, and volunteer service to make communities safer, stronger, and better prepared to respond to the threats of terrorism, crime, public health issues, and disasters of all kinds. All over America, communities have organized Citizen Corps Councils to inspire people to take action and get involved.

Caleb Gilbert
Public Health Advisory Council Coordinator
Caleb Gilbert is the Public Health Advisory Council Coordinator for the Carroll County Coalition for Public Health and has been with Granite United Way since September 2018. Caleb has worked in the public health field since 2014, working for state government and local nonprofit programs. He grew up in neighboring Maine, enjoying the outdoors, and is excited to serve in the Granite State. Caleb holds a Master’s degree in Social Work from the University of Southern Maine School of Social Work in 2015, focusing on community social work and systems level approaches to problems facing communities. He is excited to use the experience he already has in the field and gain invaluable new experiences working in Carroll County. Catalina Kirsch
Continuum of Care Facilitator
Catalina has worked in community education, public health and youth development for over 30 years. She holds a Master of Education degree and is a NH Certified School Counselor. She has led efforts to promote wellness and advocate for children, youth and families in Carroll County in public schools, after school programs, early childhood and public health settings. She is an active volunteer in the local outdoor community. As a member of the C3PH team, she works in Carroll County to increase awareness of and access to services; to improve communication and collaboration among service providers; and to build capacity and expand delivery of services related to substance use disorder. Lindsay Richardson
Public Health Emergency Preparedness Coordinator
Lindsay has worked within the public health space since 2004, focusing primarily on addressing and meeting the mental health needs of children, adolescents, and families. Within the last few years, her interests became more focused on Emergency Preparedness and Response, and she has attended numerous trainings with the Federal Emergency Management Agency (FEMA) and the Texas A&M Engineering Extension Service (TEEX), where she is currently pursuing certification as a Community Disaster Preparedness for Healthcare Professional. Lindsay also holds a Master of Science degree in Clinical Mental Health Counseling. In her role as PHEP Coordinator, Lindsay works to support the development and coordination of regional capacity and response regarding a variety of priority areas, including rapid deployment of assets to communities for all-hazard threats, vaccination services, and community education concerning public health threats. Soyla Hernandez
Community Health Worker
Soyla Hernandez is the Community Health Worker (CHW) for Carroll County Coalition for Public Health. A CHW is a frontline public health worker who is a trusted member of the community that serves as a link between members of the community, healthcare systems, and social services. CHW’s can provide support, education, and guidance to help you access the care and resources you need. Soyla has been with Granite United Way since October 2022. She works in Carroll County to increase awareness of and access to resources to improve the well-being of individuals in the community. She has worked as a Texas state Certified Community Health Worker since 2014, working in healthcare for state government and local nonprofits since 1999. She loves family time and traveling. Funding Sources
The Carroll County Coalition for Public Health is an initiative of Granite United Way and supported by diverse funding sources. These resources enable us to implement effective health initiatives in our community.
Carroll County Coalition for Public Health (C3PH), an initiative of Granite United Way, is made possible through the generous support of a diverse mix of public and private funding partners. Our work is also funded through federal and state grants through the New Hampshire Division of Public Health Services, the NH Bureau of Drug and Alcohol Services and the New Hampshire Charitable Foundation with additional support from Granite United Way donors and other community partners. This blend of local, state, and national support allows C3PH to serve Carroll County, NH residents across a wide range of public health initiatives.
